To ensure your application process is easy to complete in one session, please gather the information below before beginning. With this information, you can complete your application in about 10 minutes and begin processing quickly and easily through Broadlume Payments.
Company Information - Business Legal Name

● For Multi-location applications, please enter each individual physical address on the applications. The Billing/Legal address can be the same, but the physical address must differ.
❒ Business Start Date
❒ Tax-Identification Number of the Business (EIN)
❒ Company Type
● Association, Corporation, Financial Institution, LLC, Partnership, Sole-Proprietor, or Tax Exempt.
❒ Website
❒ Primary Owner Information - Name and Title
❒ Address

❒ SSN
● Required by the Patriot Act to ensure we verify the identity of the person(s) opening a merchant account.
● After submitting the application, please unlock your Experian credit for 48-72 hours for our Underwriting team.
❒ Ownership Percent
● Anyone who owns 25 percent or more of the organization should be included in the application.
Banking Information
❒ Name on the Account
● The bank account must be tied to the legal entity and must be a business checking account that can be both debited and credited.
❒ Account and Routing Number
● For Multi-Location Applications, please ensure the correct account is correlated to the application if different.
❒ Most Recent 3 months of CC processing or Bank Statements
❒ Annual Volume
● For Multi-Location Applications, please enter the annual revenue for the individual location. Do not group the volume on each application. (i.e., if you have 3 locations with a total volume of $10 million under the same legal entity, please break down how much each location is processing).
❒ Average Ticket Size (should not exceed High Ticket Size)
❒ High Ticket Size - The largest CC or ACH transaction you regularly receive.
